TREATMENT OF ISCHIAL SORE WITH SUBTOTAL ISCHECTOMY AND HAMSTRING MUSCLES TRANSFER

Abstract
Bedsore occurs frequently among patients with the lower half of their bodies paralysed due to traffic accidents, falling accidents or origins of various causes. The frequency of bedsore development, in the beginning, is similar for the ischium, sacrum, and greater trochanter; however, the ischial bedsore predominates as the patients do their daily activities on wheelchairs during the convalescent period. Therefore, the operation for ischial bedsore should include complete ablation of region affected by the bedsore, removal of bony prominence and enough padding by the tissue that has good blood supply. By using the hamstring muscles, the author treated the four cases of ischial bedsore in patients with the lower half of their bodies paralysed and obtained the following conclusions:

1. The operation is very simple, and it puts less burden on paients both psychologically and physically.

2. Enough padding can be accomplished.

3. The recurrence was rare.

4. It is more useful especially in the case where the recurrence is due to other surgical method.

5. The limitation is that it is applicable only in patients with the lower half of their bodies paralysed.
